摘要:
利用纳米细菌纤维素膜修复兔颈段食管缺损,探讨细菌纤维素作为人工食管的可行性。制备15例兔颈段食壁长为1 cm,环1/3~1/2周径的全层食管壁缺损的动物模型,利用细菌纤维素膜修补食管缺损。术后观察兔的生存、生活状况,并于术后第8、12 w分别取材,对血液学及移植物行大体、组织学检查,并在术对前每例动物行X线检查。实验中2只发生吻合口瘘死亡,1只因为消化系统疾病死亡,1只术中死于气胸,共存活11只,其术后8~12 w体重及白细胞计数恢复至正常水平; X线检查表明所有实验组兔食管均未见明显狭窄及梗阻征象,与正常未手术兔相比未见异常。组织学观察表明术后8 w,食管缺损处大部分覆盖了新生的鳞状上皮细胞,细胞层数逐渐增多,分化逐渐成熟,至12 w基本实现了再上皮化。BC膜能够提供良好的上皮再生环境,可能是一种有前途的人工食管材料。
Abstract:
To study the possibility of bacterial cellulose used as artificial esophagus by repairing partial esophageal defect of rabbit.15 adult rabbit were used for model of partial esophageal defect of 1 cm long and 1/3~1/2 perimeter and repaired by bacterial cellulose and autogeneic fascia of cervical segments respectivly.The survival and living states of all animals were observed during the whole experiment process, and the gross appearance, histological appearance were observed at weeks 8 and 12 post-operation and X-ray films was observed in each rabbit before the operation.11 experimental rabbit survived over 12 weeks,two died of anastomotic leak, one died of the disease of digestive system and one died of pneumothorax in experimental group,the weight changes and WBC counts had no difference between the experimental group and control 8~12 w post-operation . Iodine imaging X-ray examination showed that no obstruction and structure in anastomotic stoma. Histological results indicated that 8 w post-operation, the internal surface of defect was covered with 3~5 layers of epithelial cells, the epithelium further differentiated into 8~10 layers and collagen fibers were observed under squamous epithelium at 8~12 weeks post -operation.BC membrane can provide a well environment for repairing of epithelium and has a promising application on artificial esophagus material.
