设计帮助医生掌握腹腔镜手术技能的训练系统并初步验证其有效性能。 基于人因工程原则设计训练器的尺寸轮廓和穿刺孔位置;依照McGill技能训练标准和腹腔镜手术基本、高级技能设计训练任务模块;根据腹腔镜手术操作过程中医生的最佳操作姿势对台车进行设计;为配合使用腹腔镜以及根据空间运动要求设计扶镜器。12名无腹腔镜手术经验的学生随机分为2组,使用腹腔镜手术训练系统分别以多孔和单孔操作的形式连续9天练习3项基本任务,记录2组人员训练前后完成任务的时间并进行比较。 2组无腹腔镜手术经验的学生在使用该训练系统培训后,完成任务的时间均显著缩短(〖WTBX〗P〖WTBZ〗<0.05)。 本研究设计的训练器系统符合人因工程学,对于熟悉腹腔镜手术器械的使用,提高多孔和单孔手术操作能力有显著的训练效果。
Abstract:
To design a set of training system for laparoscopic skills and preliminarily evaluate its usefulness. The size, contour and port location of the training box were designed based on ergonomics. Training tasks were designed according to the McGill Laparoscopic Simulation study and basic, advanced laparoscopic skills. Trolley was designed according to the optimal posture for the MIS surgeon. Taking mirror device was designed according to the space motion requirement for using laparoscope system. Twelve novice students were randomly divided into A,B two groups, they accepted consecutive nine days of trainings to practice three tasks in single and multiple ports using the laparoscopic training system. The time of completing tasks before and after the training was recorded and compared. A set of laparoscopic training simulation system was successfully designed. It was mainly composed of training box, trolley, monitor, help mirror device. The time of completing tasks of two groups of novice students after training was significantly shortened (〖WTBX〗P〖WTBZ〗<0.05). The training system is ergonomically designed, it is useful for improving multiple and single laparoscopic operation ability.
