为了对不同内固定系统参数在不同愈合时期下对内固定系统的影响进行评估,本研究通过对一名健康志愿者胫骨进行 CT 扫描,利用Mimics、Geomagic studio、Abaqus 等软件建立胫骨骨折钢板内固定实体模型,采用正交试验设计的方法对不同因素水平进行组合,观察加载后不同组合的接骨板、螺钉上的最大等效应力及内固定系统的最大轴向位移。结果表明,内固定系统在愈合初期与愈合后期的力学环境差异较大,愈合初期过早负重易导致强度失效而造成断裂,愈合后期的断裂是由于疲劳所引起的;不同愈合时期,不同因素对不同评价指标下的影响主次顺序也不同,接骨板及螺钉的尺寸对内固定系统的影响最大,螺钉布局中螺钉数量及接骨板工作长度也产生较大的影响。

To estimate the effects of internal fixation parameters on the internal fixation system under different healing period, a solid model for finite element analysis(FEA) was set up by the software of Mimics, Geomagic studio and Abaqus, with the data obtained from CT scanning for a healthy volunteer. Different combinations of internal fixation parameters were constructed with the usage of Taguchi method, the maximum equivalent stress of plate and screws and the maximum axial displacement were resulted from the FEA. Results showed that there exist a big difference between the early and late healing phase of internal fixation system, from which we can conclude that the fracture of internal fixation is caused by the strength and fatigue failure in the early and late healing period respectively. Moreover, the important order of factors are different, the size of plate and screws has the greatest influence on the internal fixation system, while the number of screws and the working length of plate also has great influence.
