摘要:
为验证本研究基于磁锚定技术的磁性水凝胶在辅助内镜下黏膜剥离术中的效果,以微米级的Fe3O4颗粒与海藻酸钠配置质量分数为50%的磁流体,以葡萄糖酸钙为离子交联剂,使海藻酸钠磁流体快速凝胶化形成磁性水凝胶。以猪离体的食管和胃为模型,借助双通道注射装置将磁流体与钙剂同时注射到待切除的黏膜,放置锚定磁体,观察术野中黏膜剥离面显露情况。经双通道注射装置可便捷地将海藻酸钠磁流体和钙剂同步注射到食管和胃黏膜内。在钙剂作用下,海藻酸钠磁流体可迅速发生凝胶化反应,形成磁性水凝胶。在锚定磁体磁场力作用下,注射有磁性水凝胶的黏膜被有效吸引,充分显露黏膜下层的剥离面,并为ESD提供清晰的术野和足够的组织张力。体外模拟实验结果显示,基于磁锚定技术的磁性水凝胶黏膜下注射可有效显露黏膜剥离面术野并提供足够的组织张力,有助于ESD操作。
Abstract:
To verify the effect of magnetic hydrogel based on magnetic anchor technique in endoscopic submucosal dissection(ESD),the magnetofliud with a mass fraction of 50% was prepared with micron-sized ferric tetroxide particles and sodium alginate.Calcium gluconate was used as an ionic crosslinking agent,sodium alginate magnetofliud was rapidly gelated to form magnetic hydrogel.The esophagus and stomach of pig as the models, the magnetofliud and calcium were injected into the mucosa to be excised synchronously with the aid of double channel injection device, and the anchoring magnet was placed to observe the exposure of the mucosal stripping surface in the operation field.The sodium alginate magnetofliud and calcium could be injected into esophagus and gastric mucosa smoothly and synchronously by two-channel injection device.Under the action of calcium agent, the sodium alginate magnetofliud could be gelated to form magnetic hydrogel.Due to the magnetic force of the anchoring magnet, the mucosa injected with magnetic hydrogel was effectively attracted, which could fully reveal the dissection surface of the submucosa, and provided clear operation field and sufficient tissue tension for the ESD.The result of simulation experiment in vitro show that magnetic hydrogel submucosal injection based on magnetic anchor technique can effectively expose mucosal dissection field and provide sufficient tissue tension, which is helpful for ESD operation.
