维持性血液透析患者由于内瘘所致的高血流量或中心静脉导管等因素,常出现中心静脉闭塞或狭窄等中心静脉疾病(central venous disease, CVD),导致血透通路功能障碍。经皮球囊扩张血管成形术(percutaneous transluminal angioplasty, PTA)和经皮血管支架植入血管成形术(percutaneous transluminal angioplasty with stent placement, PTS)是透析相关CVD的主要治疗手段。本文首先阐述了透析相关CVD的临床背景,指出使用支架治疗CVD的有效性;再从不可降解和可降解材料两个方面分别回顾了血管支架的发展;最后指出用于静脉和动脉疾病治疗支架的区别,介绍了目前临床上专门用于静脉的血管支架,并对未来静脉用支架的发展进行展望。

Central venous disease (CVD) is a common and intractable complication in patients on maintenance hemodialysisdue to the factor like high-flow status infistula and peripherally inserted central catheter. Percutaneous transluminal angioplasty (PTA) and percutaneous transluminal angioplasty with stent placement (PTS) are recommended as the main treatment strategy for symptomatic CVD.In this paper, the clinical background of dialysis-related CVD and the therapeutic validity of stent are introduced. Then, the development of vascular stents is reviewed from the aspects of non-degradable materials and degradable materials respectively. Finally, the differences of stents used in the treatment of venous and arterial diseases are pointed out. The existing clinical vascular stents specially used for venous stents are introduced, and the future development of venous stents is prospected.
