本研究采用Mimics和Solidworks软件对患者真实冠状动脉CT影像数据进行建模,获取左冠状动脉几何模型。基于双向流固耦合对左冠状动脉内的血液流动进行数值模拟,获得正常血压、轻度、中度和重度高血压四种工况下冠脉内血液流场,研究了冠脉内血压的脉动变化,分析高血压对冠状动脉粥样硬化斑块的影响。结果表明,血压变化导致冠状动脉内血液流场发生改变,在斑块后出现血液涡流紊乱现象,TAWSS小于0.4 Pa和RRT大于8 Pa-1区域出现在狭窄斑块后方和血管分叉附近。随着血压升高冠脉内血液流速降低,低TAWSS和高RRT区域面积增大,尤其在中度至重度高血压区间内变化幅度最大,表明高血压会加速冠状动脉斑块发展。
Abstract:
We used Mimics and Solidworks softwares to build a geometric model of the left coronary artery based on the patient’s real coronary CT image data. Numerical simulation of blood flow in the left coronary artery was studied by two-way fluid-structure interaction method. The effect of hypertension on coronary atherosclerotic plaque was analyzed by using the intracoronary blood flow field under four working conditions: normotension, mild, moderate and severe hypertension. The results indicated that blood pressure caused significant changes in the blood flow field in the coronary arteries and vortexes appeared in the back position of plaque. Regions of TAWSS < 0.4 Pa and RRT>8 Pa-1 in the model occured in the back position of narrow plaque and bifurcation position of vessel. The intracoronary blood flow rate decreased and the areas of low TAWSS region and high RRT region became larger with the increase of blood pressure, especially, the largest change occured in the range of moderate to severe hypertension. It indicates that hypertension will accelerate the development of coronary plaques.
